    The main documents required for any visa application for a short visit:

     

     ● Visa application form filled out in Polish or English bearing personal signature of the applicant (completed and signed). A registration and filling out the visa application forms should be made thtough electronic customer service system"e-konsulat":http://www.e-konsulat.gov.pl/ (visa application forms which was not registered viahttp://www.e-konsulat.gov.pl/ won't be accepted). Minor's visa application form is signed and submitted by the parent or legal guardian. It means, that for the minor entering in the parent's or legal guardian's passport should be submitted a separate visa application form, minor receives a separate visa in the parent's or legal guardian's passport. Visa application forms can be obtained free of charge: at the Embassy's office or website, You can make a registration and fill out the visa application forms via customer service system on http://www.e-konsulat.gov.pl/        

     

    ●  color photograph (1 pcs.) meeting the following requirements:  be in color, in sharp focus, on white background and with good contrast,  3,5 x 4,5 cm, printed on high quality photo paper with high resolution,  be    maximum 6 months old and resemble the applicant, be taken with the applicant facing the camera directly, showing distinctly eyes and face from both sides from the top of the head to the top of the shoulders. The face must occupy 70-80% of the picture, shows a person without a headgear, facing the camera directly with closed mouth, hair must not cover the face.

     

    ●  Passport, signed and valid for at least three months after the last intended date of departure from the territory of the EU-Schengen Group Member States, the passport must contain at least two blank pages (for Visa) and must have been issued within the previous 10 years. All valid international passports in original and expired one if available. Continued passports will not be accepted. Passports are accepted without any wrappers such as plastic, leather. Please attach copies of pages in the passport with the previous Schengen visas. 

     

    ● Proof of sufficient financial means if not covered by host can be provided in form of: employment  certificate  confirming the position of employee and salary within a period of 6 months and year of employment, letter from bank confirming deposit account and amount available, letter from bank confirming holding of credit card and card limit, account statement for last 3-6 months, traveler checks, in case of travelers depending on others for covering trip expenses the Embassy may accept a guarantee letter from person paying for the trip and proof of financial means from person providing financial support.      

     

    • Medical insurance with €30 000 coverage valid in all EC countries. In case of one entrance - medical policy should cover all days of the planned stay, in case of multi entrance - medical policy should cover first stay of the planned trip. Travel medical insurance (period of validity should exceed 15 days more than intended trip).
    • Original employment certificate with book employment:

    - For employed persons: employment certificate confirming the position of employee and salary within a period  of 6 months and salary within a period of 6 months and year of employment.
    - For private entrepreneurs:  copy of the license/registration of company.
    - For pensioners: pension certificate.
    - For students: confirmation from university or educational establishment.

     

     ●  Copy of the ticket reservation.

          Additionally for Minors (under 18) that are travelling without or with one parent only: written consent from the parent/parents (not travelling with the child) is needed. If there is only one custodian, decision from court about sole custody, death certificate or other document has to prove this.  

     ●  Consular fee for processing visa applications is 300 AED.  

     

    Additional requirements:

     

    Applicants, who are not UAE citizens have to additionally pro vide a copy of the valid UAE resident visa.

     

    A)  For members of official delegates:

     

     ●   Letter in original or note of the UAE government bodies on confirmation that person who make a claim is in complement of delegation which is inviting to Poland (the surname must be point out in the letter).


    B)  For business trips:

     

     ●  Written request (invitation) from host company. The invitation letter should have the date, signature.

     ●  The invitation letter should have the following information:

     ●  Name of inviting company;

     ●  Full name, year of birth, sex of the invited person, as well as his/her job title;

     ●  Purpose of visit;

     ●  Duration of visit (dates, number of days and entries);

     ●  Who bears the costs of traveling and accommodation;

     ●  Inviting party address and telephone number, job title, full name, telephone of the person responsible for  traveling arrangements.  

     

     C) For persons, who take part in scientific, cultural events and art, and also in university's and in other programs of exchanges: 

     

     ●  Written request (invitation) from host non-profit-organization.

     ●  Confirmation of non-profit-status of host organization  continuing, information about side, which cover expenditure.

     ● Confirmation from the place of employment or the evidence of the membership of the Emirate's scientific, cultural organization, which is a member of the relevant program.

     

    D) For pupils, students, students after after-diploma education and teacher, who accompany them with aim of studying, taking part in trainings, taking part in frames of programmer's exchanges and others education events:

     

     ● The original document or reference about entering to the university, college, or school, or reference   from courses with information about kind, term of studying and about person, who cover all expenditure. 

    E) For the competitors in international sports competitions and persons,  who accompany them:

     ● Original of written request (invitation) from host non-profit-organization, list of competitions with  conforming date of birth and rank.

     ● The document which confirm fact that person is a sportsman and member of sports  organization.

     

    F) For private invitation trips - visits to relatives or friends:

     

    ● The original and copy of invitation letter registered at the local authority in Poland (confirmed by the  Voivodship office).

    ● The written In the case of the lack of kinship with a inviting person, in addition a written request containing as follows:

       - Full name, address and telephone  number of the Host.

       - Full name, address, date of birth and passport number of the Applicant.

       - Date, duration and destination of trip. - Information on relationship between the host and applicant.

       - Information on who undertakes traveling costs.

       - reference from the place of employment/study indicating position, employment/training acceptance date, net salary, etc.).  

     

    G) For the treatment,

     

    • Original document from the medical establishment with a proof of medical services in that medical establishment and with information about treatment and possible medical expenses.
    • Proof of sufficient financial means, which covers the cost of treatment.

    H) For tourist trips:

     

    ● In case of organized tourism (visa accredited travel agencies): booking confirmation of organized trip from host company (i.e. voucher). Voucher or a letter from the Polish travel agency, confirming the hotel reservation and contains the following information: date of the reservation, names of the hotel reservation, an address and telephone number of the hotel. The hotel reservation should be partially or fully paid or booking via Internet, where will be given information about payments by a credit card.

    ●  In case of individual tourism: confirmed hotel reservation with down-/prepayment

    ●  Travel plan.

     

    I) Transit visa:

     

    • Original and copy of valid visa of target country.

    If you don't need a visa to the country of destination, you have to confirm the evidence of destination.

     

    These documents are sufficient for the issue of visa, however, the Embassy may request for an additional  documents  in its sole discretion.

     

    The Polish Embassy in Abu Dhabi has a right to invite an applicant for an interview.

    Submission of forged documents and giving the evident false information, as a result, may be caused a  prohibition on entry to the Schengen Agreement countries.

    Required documents for a visa should be submitted not earlier than 3 months before the scheduled departure.

     

    You cannot have two valid Schengen visas simultaneously.

     

    Time consideration of visa application forms

     

    The decision about the visa is considered within 10-12 working days from the date of submission the complete visa application. The visa application form Has to be previously registered and filled out on website system „e-konsulat": http://www.e-konsulat.gov.pl/ If there is a necessity of detail inspection of the documents, the process could be extended up to 30 days. In some urgent cases the decision can be taken during 5-7 working days or earlier before scheduled departure in condition of registration and filling out the visa application form via Internet system http://www.e-konsulat.gov.pl/.

     

    Valid visa doesn't guarantee the entrance to the territory of the EU-Member States.

    The Border Guards determines about times of entrance.

     

    If you intend to enter Republic of Poland with a national visa "D" enabling you to stay in Poland for longer than 90 days, it is recommended to apply for visa in advance - not less than 10 working days before the trip is planned.

    ● Proof of sufficient financial means can be provided in form of: employment  certificate  confirming the position of employee and salary within a period of 6 months and year of employment, letter from bank confirming deposit account and amount available, letter from bank confirming holding of credit card and card limit, account statement for last 3-6 months, signed traveler    checks, invitation (original and copy), issued by the Voievoda Administration according to  the law on foreigners up to the 13 th of June 2003.

     

    • Medical insurance with €30 000 coverage valid in all EU countries (original and copy). Medical insurance should be type scripted. In case of application for foreigner's employment medical insurance should be bought for the whole staying period.

    ● Copy of the ticket reservation.

    ● Additionally for Minors (under 18) that are travelling without or with one parent only: written consent from the parent/parents (not travelling with the child) is needed. If there is only one custodian, decision from court about sole custody, death certificate or other document has to prove this. 

    ● Consular fee for processing visa applications is 300 AED.

    We inform you, that EU-Statement № 265/2010  of  Europarliament and the Board that makes  alterations in the Schengen Executive Convention and the Statement №562/2006 concerning personae movement having long-term visas:  

     

    ●   All the visas of type D that allow to move across the Schengen zone during 3 months in every 6 months period - are the same as the guest visas.

    ● States-Members can issue visas of type D maximum for  one-year period.

    Visas of  type D issued up to the 5 th of April, 2010 having  longer period of action   should be substituted for  the Letters of  Permission  for staying in the country during a year beginning from the 5 th of April, 2010
